By attending any class, event, or other program at St. Francis of the Woods, you are agreeing to our Photo Release. Please make sure you read and understand this Photo Release before completing your enrollment or attending events at St. Francis of the Woods:

I hereby grant St. Francis of the Woods permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. This permission also extends to photographs, videos, or other digital media (“photo”) taken of any child I enroll in any program or event via StFrancisoftheWoods.org.

I hereby irrevocably authorize St. Francis of the Woods to edit, alter, copy, publish, exhibit, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness or the likeness of my child(ren), heir(s), and/or ward(s) appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge St. Francis of the Woods from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

BY ATTENDING OR ENROLLING IN ANY CLASS, EVENT, OR OTHER PROGRAM AT ST. FRANCIS OF THE WOODS, I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE AND HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE.

If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. How long we retain your data
